The FDA K161950 public 510(k) summary lists 31G common-type models at 0.25 mm outside diameter and 32G common-type models at 0.23 mm outside diameter.[1] In that documented family:

The K161950 dimensions document named models from one manufacturer. A buyer should use the current drawing, specification sheet and regulatory mapping for the exact quoted REF before approving another brand or private-label version.

4. Gauge and Needle Length Are Separate Variables

Search results often compare a 32G × 4 mm product with a 31G × 5 mm or 31G × 8 mm product. That can be useful when comparing the two complete SKUs, but it does not isolate gauge because both thickness and length have changed.

The K161950 common-type model list includes both 31G × 4 mm and 32G × 4 mm, as well as both gauges at 5 mm. It lists a 31G × 6 mm common model but not a 32G × 6 mm common model.[1] That does not mean 32G × 6 mm is unavailable everywhere; it means this one public file cannot be used to establish coverage for it.

5. Does a 32G Pen Needle Hurt Less Than a 31G?

A thinner needle can support a lower-pain design, but “32G is pain-free” would be inaccurate. Perceived discomfort is influenced by the whole device and the use event, including tip geometry, penetration force, lubrication, needle length, base pressure, injection site, technique, tissue characteristics and prior experience.

What the current expert recommendation says

The 2025 FITTER Forward device-characteristics infographic recommends 32G as the width for pen needles and states that current evidence suggests 32G can reduce pain without being too fragile. The same document separately highlights needle-wall thickness, bevel geometry and base design, which is important: the recommendation is not based on gauge in isolation. [2]

Why an older head-to-head study does not isolate gauge

A randomized crossover study compared a NovoFine 32-gauge-tip 6 mm product with a Micro-Fine Plus 31G × 5 mm product and reported better ratings for several user-perception outcomes with the tested 32G-tip product.[3] However, the products also differed in length and needle geometry. The result supports the tested device comparison; it cannot prove that changing only the gauge from 31G to 32G will reproduce the same outcome in every product family.

Safer wording for a buyer-facing page

Use “32G has a smaller outside diameter, and current evidence supports 32G as a recommended pen-needle width.” Avoid “painless,” “pain-free,” “guaranteed comfort” or “always more comfortable.”

6. Gauge, Inner Lumen, Wall Thickness and Flow

Gauge commonly communicates outside diameter. Medication flows through the internal lumen. These are related dimensions, but they are not the same measurement.

7. Is a 32G Pen Needle More Likely to Bend or Break?

It is reasonable to ask whether a thinner cannula changes handling or mechanical margin. It is not reasonable to conclude that every 32G pen needle is weaker than every 31G pen needle. Material, wall, length, tube processing, hub fixation, insertion conditions and applicable performance tests all matter.
